What you will need:
The following materials should be sent REGULAR mail.  Please do not send as certified mail, as we are not here everyday to sign for it.

Ø  Application Form

Ø  Additional Information Form
Ø  Psychological Evaluation
Ø  Educational Evaluation
Ø  First 2 pages of the IEP (if child has one)

Somewhere on the application be sure to indicate the Center to which you are applying.

GUIDELINES FOR ACCEPTANCE
In order to determine if our program is appropriate for your child, all applicants must complete application forms and provide copies of recent evaluations (psychological and educational) conducted by the Child Study Team and/or an Independent Diagnostician.

After information is received our professional staff reviews your application paying particular attention to whether the design of our program would be appropriate for your child.  If that is the case, you will be notified that your child has been placed on the Waiting List for the Center of your choosing.
